About Extended
Extended Home Care has been providing specialized home-based care since 1997.
We are one of the few special needs Certified Home Health Agencies (CHHA) approved by the Department of Health (DOH) to serve populations eligible for services from the Office of Mental Health (OMH) or Office for People with Developmental Disabilities (OPWDD).
We partner with families, schools, Care Coordination Organization (CCO) Care Managers, OPWDD providers, and other agencies to helps patients of any age get the in-home services they need. Extended provides coordination beyond admission, ensuring the maximum support and resources needed for comprehensive, ongoing care.
